Zofenopril – unikalny inhibitor konwertazy? Artykuł przeglądowy

Abstrakt

Zofenopril jest lipofilnym inhibitorem konwertazy zawierającym grupę sulfhydrylową w cząsteczce. Lek łatwo penetruje do tkanek, gdzie w procesie deestryfikacji ulega przekształceniu do aktywnej cząsteczki zofenoprilatu. Udowodniono, że w porównaniu z innymi inhibitorami konwertazy ma on dużą tkankową aktywność w hamowaniu enzymu konwertującego. Efekt ten utrzymuje się szczególnie długo w sercu i łożysku naczyniowym. Zofenopril wywiera korzystny wpływ na funkcję śródbłonka, działa przeciwzapalnie, zwiększa lokalne wytwarzanie tlenku azotu i hamuje metaboliczne oraz hemodynamiczne efekty niedokrwienia mięśnia sercowego. Skuteczność leku potwierdzono w badaniach na chorych na nadciśnienie tętnicze oraz chorych z zawałem serca z niewydolnością serca i bez niewydolności serca.
