Does concomitant aspirin use reduce effectiveness of angiotensin-converting-enzyme inhibitors after myocardial infarction? Review article

Abstract

Aspirin and angiotensin converting enzyme (ACE) inhibitors are widely used in cardiology. Based on retrospective analysis of large clinical trials, researchers have suggested that aspirin may reduce the effectiveness of ACE inhibitors in patients with coronary artery disease or heart failure. The data is not sufficient to create a recommendation not to combine these drugs. A prospective randomized study SMILE-4 showed sustained positive effects of the ACE inhibitor, zofenopril which was used together with aspirin in acute myocardial infarction. Likely special properties of zofenopril, sulfhydryl containing ACEI are the cause of the persistence positive cardioprotective effect even in presence of cyclooxygenase inhibitors.
