New recommendations of management of Patients With Extracranial Carotid and Vertebral Artery Disease Review article

Abstract
Article summarizes newest data and recommendations concerning carotid and vertebral artery stenosis. The prevalence of carotid artery stenosis is about 50% in population of patients after stroke. Carotid endarterectomy in patients with 50–90% of stenosis reduces significantly risk of stroke. A new form of therapy is carotid artery stenting particularly useful in patients with high clinical risk or with anatomical disorders. In older patients with anatomic complexity or atheroma carotid endarterectomy is reasonable choice. Carotid artery stenting may be advised in younger patients, in those with recurrent stenosis, contraindications for endarterectomy, after carotid area radiotherapy or in those with carotid anatomy inaccessible for surgery. Optimal non-invasive therapy is an efficacious alternative in therapeutic approach. In all patients lifestyle modifications, therapy with antiplatelet drugs and control of risk factors are necessary in order to improve results and decrease the risk
